Rates and Insurance
Rates vary depending on session lengths and modalities.
Services may be covered in full or in part by your health insurance or employee benefit plan. We will gather your insurance insurance at the time of scheduling your first appointment in order to obtain a quotation of benefits for you.
We will also bill your insurance directly, whether or not we are preferred providers for your insurance as a courtesy to you.
All co-pays are due and payable at the time of service.
Your plan may also have out-of-network benefits that will cover our services, so please do not be deterred if your therapist of choice is out-of-network with your insurance.
What is most important is that you feel safe and comfortable with your therapist. Payment for services can be negotiated from there.
OptimalLife Wellness Center is preferred with the following insurances:
Kaiser Permanente (formerly Group Health) PPO
Optum Behavioral Health
Premera (Blue Cross)
Wellpoint Behavioral Health
Please note: individual clinicians may or may not be preferred as of yet with your insurance. If your preferred clinician is not preferred by your insurance company, you may still be eligible for out-of-network benefits through your insurance. We will work with you to get our services covered as much as your insurance company will allow.
We will bill your insurance directly, so that you won’t have to.
We are also covered by several EAP providers, please check with your human resources professional.
Although we will check your benefits for you, it is still your responsibility to check with your insurance as to what your plan does and does not cover. We will make every attempt to collect from your insurance. However, you should be aware that you are responsible for services rendered not paid by insurance within 90 days. Here are some helpful questions to ask your insurance carrier:
- Do I have mental health benefits?
- What is my deductible and has it been met?
- How many sessions per calendar year does my plan cover?
- How much does my plan cover for an out-of-network provider?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Sliding fee services are available on a limited basis. Please inquire directly at time of placing your first appointment.
Cash, check and Visa/Mastercard/Discover/Amex credit cards accepted for payment.
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay a late cancellation/no-show fee equal to the amount of the session rate reserved for you.
